Job Description
Initial Assessment of Needs: Case Managers will interview new clients, or parents or guardians, and explore their needs. CM identifies what outside agencies the client may also be utilizing. Cm works with the client to develop an Individual Treatment Plan the client agrees upon.
Monitoring: Behaviors and symptoms, thought processes, activities of daily living, parenting skills. Maintain appropriate documentation and reports.
Linking: Community resources, housing, treatment providers, medical specialists, legal assistance, and ombudsman.
Coordinating: Treatment providers, outside resources/agencies, medical providers, landlords.
Advocacy: outside resources, legal system, custody issues, landlords, ombudsman.
Maintain professional standards regarding practice, conduct, continuing education, appearance and relationships with clients and other professionals.
Other duties as assigned.
