FLSA Status: Exempt
FTE: 1 FT 40 hours/week Flexible Schedule to meet the needs of the sites assigned; may include weekends as needed
Pay : Starts at $40/hour= 83,200 exempt depending on qualifications, experience in the field, skills and education acquired

Your Impact:
Perform medical patient assessments and assist in developing individualized care plans
Advise and administer medications, treatments, and therapies, as prescribed
Monitor patient progress and provide ongoing evaluations and updates to the care team
Educate patients and unlicensed direct care employees on health management and home care procedures
Coordinate care with other healthcare professionals and community resources
Maintain accurate and timely documentation of patient care and progress
Perform other duties as assigned, contributing to the success of the program and the overall mission of the organization
What you'll need:
Valid RN license in Minnesota
2 years of nursing experience or equivalent, preferably in home health or community health settings.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Department of Human Services background clearance
Valid driver's license, reliable vehicle, acceptable driving record
Ability to effectively communicate in English, both written and verbally
