Our interdisciplinary team helps seniors thrive and now we are seeking a dedicated, forward-thinking Physician (MD/DO) to join our expansion team.

You will manage a smaller panel of our most complex patients, supported by an exceptional care team, with the time and tools you need to deliver meaningful care.

Your Role:

• Deliver medical care via telehealth and in the patient's home.
• Perform comprehensive assessments and manage complex conditions.
• Lead end-of-life and goals-of-care discussions with compassion.
• Coordinate care with families, medical decision makers, and other providers.
• Educate patients and caregivers to promote better health outcomes.

Experience and Qualifications:

• Board-certified MD/DO in Geriatrics, Family Medicine, or Internal Medicine , with at least three years of experience.
• Passion for working with seniors; home-based care experience is a plus, but we will train the right person.
• Active, unrestricted medical license and DEA certificate.
• No history of license or controlled substance disciplinary actions and not under current investigation.

Schedule and Compensation:

• Work two to four days per week we will design a schedule that works for you.
• No nights! No weekends!
• Competitive salary plus incentive and bonus program, commensurate with experience.
