We have a mission driven and supportive culture. We are seeking a like minded individual to join our team as an outpatient primary care / adult medicine RN M-F 5x per week.

Job Details

M-F 5x per week seeing patients in an outpatient seeing.

What you will be doing:

• **Patient Assessment**: Conduct comprehensive assessments of adult patients, including medical history, vital signs, and current health concerns.
• **Care Coordination**: Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
• **Patient Education**: Provide education to patients and their families about health conditions, medications, and lifestyle modifications to promote wellness and manage chronic diseases.
• **Clinical Procedures**: Perform various clinical procedures such as administering medications, wound care, and assisting with minor surgical procedures.
• **Documentation**: Maintain accurate and timely documentation of patient care, including electronic health records (EHR) and other required reports.
• **Follow-Up Care**: Monitor patient progress, provide follow-up care, and coordinate referrals to specialists or other healthcare services as needed.

Skills you need:

• RN license
• Outpatient experience highly preferred
