Employment Type: Full Time | Monday - Friday, 8a-5p
Salary Range: $68,000+/ annually (pay is based on requirements and experience)
PLUS $3,000 Sign-On Bonus **
Our RN Case Manager will oversee and coordinate the care of our PACE Participants. In this role you will assess participant needs, develop tailored care plans, monitor progress, and collaborate with healthcare professionals to promote optimal outcomes.
Key Responsibilities:
Conduct comprehensive nursing assessments and develop individualized care plans in collaboration with the IDT (Interdisciplinary Team).
Monitor participants’ health status, ensure timely interventions, and communicate changes with the care team and families.
Advocate for participant needs while ensuring compliance with PACE guidelines and organizational policies.
Document assessments, care plans, and care coordination activities accurately and timely in the EMR system.

Uniform Allowance Program
Required: Current unrestricted State of Kansas License to practice as a Registered Nurse. Availability of reliable transportation is required. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment. Must be self-directed, motivated and flexible. Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position. Preferred: Home Health, PACE, or Hospice experience.
