Fellowship House is growing treatment services! We are seeking a Wellness Coach to add to our expanded outpatient program, Promoting the Integration of Primary and Behavioral Health Care (PIPBHC). Wellness Coach will collaborate with clients and treatment team with co-occurring and physical health concerns as they transition from residential treatment and access treatment from outpatient services. Wellness Coach will utilize lived experience to provide support for clients with SUDS and MH diagnosis. Position is M-F 8-5pm with the possibility of one evening. Provides support services, information, education and support for clients. Promotes wellness, recovery and skill building to sustain better health. Provides prevention training for managing stress and problems, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. Assists clients with personal recovery goals, assisting them in developing a natural support systems, and coaches clients to advocate for themselves within their healthcare systems. Essential duties include establishing relationships and practicing core coaching skills to transform client values/desires into action and maintain lasting change over time; providing group and/or individual training/interventions focusing on improvement of the client’s functioning and health-related issues; providing wellness consultation and assisting clients with developing their Wellness Recovery Action Plan (WRAP); empowering clients to self-manage chronic health conditions and solidify wellness goals; linking clients to community-based social support/services and developing collaborative relationships with other departments, services, and community health care agencies; providing ongoing case management services; providing transportation assistance; conducting intake and assessments; assisting with discharge plans and follow up services. Knowledge, skills and abilities include effective communication, customer service skills, and problem-solving. Qualifications include an Associate’s degree and 1 year of participation in own personal recovery process or equivalent work experience, with experience providing mental health services preferred. Ability and willingness to obtain credentialing if required. Physical requirements include standing, sitting, squatting, kneeling, bending, driving, and lifting at various frequencies. Additional duties may be assigned as needed.
